I don't know if the kit comes with one or not...I'd assume it would.  The size is really going to depend on the amount of boost you plan to use and the size of the turbo.

No, his kits don't come with bov's, you will have to get one.  He likes the Greddy Type S bov, that's what the flange on the upper intake pipe is sized for.  They are not to much, I got mine off ebay for about $150.

seen it on one kit, turned out ok, fitment wasn't as great, and the manifold and placement is a bit awkward, depending on the turbo you get though you can make 220hp or so. At least those are the numbers I've seen.

IIRC, the F-max kit is rather decent. it has some quirks like the placement of the turbo. I think it sits on top of the manifold as opposed to under. this allows for the oil drain to tap into the oil separator without an auxillary pump. Like any other kit though, it does require tuning from someone with know-how. I would also consider buying a J&S safeguard.

and yes, I was talking about focus-power Tom. ;)

TXFO, I may have asked you this before, but have you read Maximum Boost by Corky Bell?  If not, you ought to pick up a copy...lots of good info on turbo's.
